Prostate Specific Antigen (PSA) PSA is a protease that makes semen more liquid by breaking down the high molecular weight protein of the seminal coagulum into smaller polypeptides. It exists in serum in multiple forms: complexed to alpha-1-anti-chymotrypsin (PSA-ACT complex), unbound (free PSA), and enveloped by alpha-2-macroglobulin. In men with prostate cancer or other prostate disorders, PSA levels are elevated and higher total PSA levels and lower percentages of free PSA are associated with higher risks of prostate cancer. The FDA first approved PSA as a screening assay for prostate cancer in the early 1990s.
