Infectious Diseases Infectious diseases are a leading cause of death worldwide, particularly in low income countries and especially in young children. The leading killers are HIV/AIDS, tuberculosis and malaria, however, lower respiratory infections (including pneumonia) and diarrheal diseases are also 2 of the top 10 causes of death globally according to the most recent survey by the World Health Organization (WHO).
Infectious diseases can be caused by several different types of pathogenic organisms including viruses, bacteria, protozoa and fungi. Transmission can occur through several different mechanisms such as contact with aerosolized droplets or infected skin, ingestion of contaminated food and water, sexual contact or through specialized hosts called vectors (e.g. mosquitoes). The prevention and control of infectious diseases has been one of the principal public health activities of this century however they continue to pose a great health problem. New diseases are continuing to emerge and growing evidence also suggests that genetic changes in infectious agents are increasing their risk of causing serious diseases. Early diagnosis can play a critical role in preventing and controlling the spread of disease by reducing its morbidity and mortality. Traditional methodologies for diagnosing infectious diseases such as microbial or viral culture and microscopy, are continually being replaced by rapid and/or more sensitive alternatives such as immunoassays and molecular diagnostics. Rapid, point-of-care (POC) IVD tests that are easy to use have been largely sought after for their convenience, portability, and quick turn-around times allowing for faster decision making in regards to treatment and disease control. Diagnostic assays that directly identify an infectious agent have become increasingly essential to

Tropical & Vector-Borne diseases
Tropical and Vector-Borne diseases are among the most complex of all infectious diseases to prevent and control. Tropical diseases typically thrive in hot, humid conditions and include infections such as Malaria, Leishmaniosis, Dengue and Chagas. Many of these pathogens are also Vector-Borne diseases, transmitted between humans by a vector which is typically a blood-sucking arthropod (e.g. mosquitoes, ticks, fleas, lice and mites). Serological point-of-care (POC) diagnostics are the most rapid and convenient methods available for tropical and vector-borne diseases. Tests either detect antigens from the infectious agent or virus specific IgG/IgM antibodies and use formats such as solid-phase dip-sticks (ELISA), particle agglutination, and lateral flow. Ideally screening assays should have minimal cross-reactivity with other infectious diseases and have a high sensitivity, especially during the early stages of the infection. There have been numerous approaches used to improve the specificity of these assays including (1) using smaller and more specific regions (sub-units) of the epitopes; (2) using multiple antigens to different epitopes within a single assay; and (3) incorporating antigens which represent common genetic variations of the disease.

ToRCH & Childhood Diseases
ToRCH is an acronym for a group of infections that can cause congenital abnormalities and it has become an important part of prenatal care to recognize these infections in the first trimester of pregnancy.
Although the organisms typically cause only asymptomatic or mild infection to the mother, they can have much more serious consequences to the fetus. If the infection occurs during the first three months of pregnancy and if it is a primary infection (newly acquired during pregnancy), the risk of congenital abnormalities is much higher compared to a secondary or reactivated infection. An important part of prenatal care is to recognize these infections in the mother and the fetus and provide suitable care. ToRCH tests measure antibodies against five groups of chronic infections and all of these infectious diseases are associated with congenital abnormalities resulting from maternal infection: • Toxoplasmosis • Rubella • Cytomegalovirus (CMV) • Herpes simplex virus (HSV) • Other infections (usually Syphilis, Hepatitis B, Coxsackie virus, Epstein-Barr virus, Varicella-zoster virus, and Human parvovirus)
For most ToRCH organisms, the initial screening test is based on the detection of antibodies to the organism. Assays are commercially available for the detection of IgG, IgM, or both IgG and IgM antibodies. In most cases, IgG reactivity in the absence of IgM reactivity is indicative of a past infection, while IgM reactivity in the absence of IgG reactivity indicates a current infection. However, for some ToRCH diseases such as toxoplasmosis and CMV, IgG avidity has recently been found to be useful for identifying primary infections. An IgG antibody produced in the first few months following an initial infection has a lower avidity than an IgG antibody produced several months or years later. Consequently, low-avidity antibody can be used to specifically identify high-risk mothers with a primary infection. To protect a fetus from ToRCH infections, early diagnosis through first trimester screening is critical.

Hepatitis
Hepatitis refers to an inflammatory condition of the liver. It is commonly caused by a viral infection from one of five unrelated hepatotropic viruses: hepatitis A (HAV), hepatitis B (HBV), hepatitis C (HCV), hepatitis D (HDV) and hepatitis E (HEV). HAV is an acute, short-term disease, which is commonly transmitted by consuming food or water contaminated by feces from an infected person. HBV and HCV are transmitted through direct contact with infectious body fluids such as blood, or semen, and typically lead to an ongoing, chronic form of liver disease. HDV is a rare form of hepatitis that only occurs in conjunction with HBV infection as the virus cannot multiply without the presence of HBV. HEV is a waterborne disease mainly found in areas with poor sanitation and typically results from ingesting fecal matter that contaminates the water supply. The disease is usually acute but can be particularly dangerous in pregnant women. Viral hepatitis is primarily diagnosed through blood tests for levels of viral antigens, anti-viral antibodies, or viral DNA/RNA. Overall, hepatitis causes more than a million deaths a year, most of which occur indirectly from liver scarring or liver cancer.

Sexually Transmitted Diseases (STDs)
Sexually transmitted diseases (STDs) are a major global cause of acute illness, infertility, long-term disability and death, with serious medical and psychological consequences to millions of men, women and infants. Today, over 30 bacterial, viral and parasitic pathogens have been identified that can be transmitted sexually.
STDs are generally acquired by sexual contact specifically through blood, semen, or vaginal and other bodily fluids. Many STDs (chlamydia, gonorrhea, hepatitis, HIV, papillomavirus, Herpes Simplex Virus and syphilis) can also be transmitted non-sexually such as from mother to infant during pregnancy or childbirth, or through blood transfusions, or shared needles. Many STDs have no symptoms, therefore an infection may go unnoticed until complications occur. STDs rank among the top five disease categories for which adults seek health care. Early and rapid diagnosis of STDs increases the chance to limit effects of the disease. Since many people infected by a STD have few or no symptoms of their infection, they put others (including unborn children from pregnant mothers) at risk. There are five main methods for the diagnosis of STDs which include (1) culture (2) microscopy (3) detection of antigens or enzymes (4) detection of nucleic acid sequences (NAAT) and (5) detection of antibodies. Of the five approaches, the assays that provide the most rapid diagnosis have gained the most acceptance. As a result, this has largely limited the traditional use of culture and increased development efforts on rapid tests using microscopy (detection of antibodies by rapid serologic methods, and specific detection of cellular components, including antigens, enzymes, or nucleic acid sequences (especially with amplification).
